Upon receipt of a completed Request for Referral
form, CCCS designated staff will date receipt of the form,
record the student’s name and all pertinent demographic data on
the Initial Flowchart, note activities on a Summary of Contact
form, and set up student’s case record according to OSE’s
procedures.

Based upon the referral form, if speech/language
is a concern the case manager shall ensure that the SLS is
invited to the Identification Meeting.

The case -manager will contact school to Request
for School Nurse Medical/Audiological/Vision Screening form to
the school’s nurse, who will:
o
arrange for an audiometric and vision screening.
o
provide the CST with the results of the
screenings/medical summary covered by FERPA with the exception
of HIV/Aids and/or substance abuse information.
o
return completed medical/audiological/vision
screening to the case manager prior to the Identification
meeting the case manager.

The case manager shall provide
parent/guardian/adult student/ and appropriate school personnel
with a copy of Notice of Upcoming Meeting form. This form will
also be made available electronically in PDF format.

Once a student has been added to the Case
manager’s caseload as an Initial case, the Notice of Up Coming
Meeting will be available. Along with this written notice, the
case manager will include as part of the information sent to
the parent a copy of Parental Rights in Special Education (PRISE).
NOTE: If the parent/guardian/adult student/ informs
the case manager that he/she has invited an attorney to be
present at an upcoming CST meeting, Cross County Clinical
Services must be immediately notified. If a
parent/guardian/adult student brings an
attorney to the meeting without prior notification, the meeting
will be delayed while the case manager contacts the legal
department. CST cannot conduct a meeting where there is legal
representation only for the parent/guardian/adult student.
